The hospice receives patients from referring institutions throughout New York City and the entire tristate area.

Summary

Assist in the collection, aggregation and analysis of data for Home Care and Hospice QA&I monitors. Responsible for data entry for intra-departmental monitoring as well as regulatory monitoring. Assists the Assistant Director of Regulatory Compliance and the Regulatory Compliance Coordinator in all educational and compliance activities.

ESSENTIAL JOB FUNCTIONS
  • Participate in the collection of data generated for the Quality Assessment & Performance Improvement program, regulatory and compliance for Calvary Home Care and Hospice.
  • Develop and format data collection tools as directed.
  • Maintain accurate and confidential records of data collected.
  • Prepares Hospice CAHPS mailing list monthly prior to submission to Press Ganey.
  • Calculate response rates for various surveys and data collection processes.
  • Coordinates the timely and accurate implementation of telephone surveys; collate responses.
  • Enters survey/QA & PI data onto appropriate computer program.
  • Prepares mailings and meeting arrangements and takes minutes for Calvary @ Home
  • QA & I and Professional Advisory Committees and Performance Improvement meetings.
  • Maintains accurate education records on each employee within Calvary @ Home.
  • Collects Medicare D information and notifies appropriate Medicare D Plan upon admission and discharge for Medicare Hospice patients.
  • Bachelor’s Degree required.
  • Minimum 2 years data entry experience required with accurate typing ability.
  • Knowledge of Microsoft Office (Word, Excel spreadsheets).
  • Knowledge of graphic display of information.
  • Attention to detail and follow-up; proven organization skills.
  • Exhibits an ability and attitude relative to the spirit of caring and tradition at Calvary Hospital as required by specific job duties and functions.
