Job Description Overview Coordinate inpatient hospice services, ensuring compassionate care and adherence to healthcare standards in a dynamic clinical environment.
Develop collaborative relationships with hospital staff to enhance patient care and streamline service delivery processes.
Conduct comprehensive assessments to determine hospice eligibility and create personalized care plans tailored to individual needs.
Collaborate with interdisciplinary teams to ensure seamless care coordination and optimal patient outcomes.
Educate patients, families, and healthcare providers about hospice services to foster understanding and support.
Provide General Inpatient (GIP) level care to hospice patients, ensuring quality service delivery and compliance.
Maintain accurate documentation of patient care, assessments, and service delivery for compliance and quality assurance.
Adapt to flexible working hours, including weekends, to meet patient needs and organizational objectives effectively.
Key Responsibilities & Duties Establish and sustain strong relationships with hospital personnel to facilitate patient referrals and enhance collaboration.
Respond promptly to referral requests and obtain necessary documentation for hospice enrollment.
Gather clinical information to assess patient eligibility for hospice services and develop appropriate care plans.
Coordinate daily hospice care for patients requiring General Inpatient (GIP) level services.
Collaborate with interdisciplinary teams to develop and implement individualized care plans.
Educate hospital staff, patients, and families about hospice services and their benefits.
Ensure adherence to state and federal regulations and organizational policies in all care activities.
Demonstrate adaptability to address referral partners’ needs and support organizational objectives effectively.
Job Requirements Associate of Science (AS) degree required; RN licensure in New York State mandatory.
Minimum of two years’ experience in Medical-Surgical or Community Health; hospice or oncology nursing preferred.
CPR-BLS certification required; valid NYS Driver's license preferred for travel needs.
Proficiency in Microsoft Office applications and effective critical thinking skills.
Ability to work non-judgmentally with diverse patient populations, ensuring inclusive care.
Strong communication, listening, and organizational skills; public speaking ability for educational initiatives.
Flexibility to work weekends and adapt to varying schedules to meet patient and organizational needs.
Experience coordinating interdisciplinary teams in a healthcare setting, ensuring collaborative care delivery.
