Job Overview

Duties
- Coordinate patient care plans by collaborating with multidisciplinary teams including nurses, social workers, physicians, and specialists across hospital departments such as ICU, emergency medicine, pediatrics, and trauma centers.
- Conduct comprehensive intake assessments and discharge planning to ensure smooth transitions of care for patients with diverse needs.
- Manage utilization review and utilization management processes to ensure appropriate resource use while complying with healthcare policies and regulations.
- Utilize EMR (Electronic Medical Record) and EHR (Electronic Health Record) systems to document patient information accurately and maintain confidentiality in accordance with HIPAA guidelines.
- Assist with case management activities, including social services referrals, behavioral health support, hospice care coordination, and working with individuals from diverse cultural backgrounds.
- Apply medical terminology knowledge along with CPT (Current Procedural Terminology), ICD-9/10 (International Classification of Diseases), and ICD coding to facilitate accurate documentation and billing processes.
- Support hospital operations by participating in patient service activities such as intake procedures, discharge planning, and working within Level I and Level II trauma center environments.
- Communicate effectively with patients and their families regarding treatment plans, insurance details, and available social services options.
- Collaborate on managed care initiatives to optimize patient outcomes while adhering to healthcare policies and compliance standards.
Experience
- Proven experience in critical care environments such as ICU or emergency medicine settings is highly preferred.
- Extensive hospital experience including acute care, hospital medicine, pediatrics, PICU (Pediatric Intensive Care Unit), OR (Operating Room), or Level I/II trauma centers is essential.
- Strong background in nursing or social work combined with familiarity in medical terminology and coding practices such as CPT, ICD-9/10 is required.
- Demonstrated proficiency in EMR/EHR systems along with Microsoft Office applications for documentation and communication purposes.
- Knowledge of HIPAA regulations, health insurance processes, discharge planning procedures, utilization review/management practices is vital.
- Experience working with individuals from diverse cultural backgrounds enhances your ability to provide personalized patient-centered care.
- Bilingual skills are considered a valuable asset for effective client communication across varied populations.
- Prior case management experience involving behavioral health or social services will be advantageous for success in this role.
