  • Size: 4-5 exam rooms (smallest site)
  • Patient Flow Daily: 15 (during their first year - they give them a ramp up for the first year on daily patient flow) Usually around 20 Patients per day after first year
  • Clinics are open from 8 to 8:30pm
Schedule
  • Three morning/daytime shifts: 8:30 AM - 4:30 PM
  • Two evening shifts: 12:00 PM - 8:00 PM
  • On - Call: 2 weeks or 1 week attached to a holiday for a year. (extra pay for this)
  • EMR
    : EPIC
Requirements
  • At least 2 years of experience of primary care exp.
  • Preferred HIV, Trans, LGBTQ etc.
  • DEA
  • Can apply Medicaid and Medicare
  • Interview Process
    : 1 round with HR and Hiring manager from the site

About the Role
Clinical Care
  • Conduct patient screenings, examinations, diagnoses, and treatments across the lifespan.
  • Provide care that is respectful, compassionate, unbiased, and consistent with evidence-based guidelines.
  • Deliver care in scheduled 20-40 minute appointments and ensure timely follow-up on patient needs, test results, and referrals.
  • Document all care promptly in the electronic health record (EPIC).
  • Provide primary care services
Collaboration & Coordination
  • Make appropriate internal and external referrals.
  • Work effectively within a team-based care model, collaborating with other healthcare professionals to ensure coordinated patient care.
Quality & Professional Development
  • Participate in quality improvement initiatives and case conferences.
  • Engage in ongoing professional development opportunities, including continuing medical education.
