Stern Therapy Consultants is hiring a PRN Physical Therapist Assistant at Bezalel Rehab and Nursing Center in Far Rockaway, NY to deliver skilled nursing rehabilitation under the direction of a Physical Therapist.

• Implement the supervising PT's treatment plan through individualized exercises and functional activities.
• Work with residents on walking, standing balance, bed mobility, and transfers.
• Reinforce safe use of prescribed mobility devices and techniques for reducing fall risk.
• Monitor treatment tolerance, follow resident-specific precautions, and report changes to the PT.
• Maintain accurate notes describing interventions, assistance provided, and residents' responses.
• Coordinate with nursing and fellow therapists to support consistent mobility practices throughout the day.

• $35–$38 per hour.
• PRN assignments based on your availability and the facility's coverage needs.
• Unlimited referral bonus, with up to $1,000 per successful referral.
• Support from therapy leadership and collaboration with the rehabilitation team.

• Active New York Physical Therapist Assistant certification.
• Ability to provide attentive care within an established physical therapy plan.
• Strong teamwork, communication, and documentation skills.
• Skilled nursing experience is helpful; new graduates are welcome to apply.
