Description
The Community Health Worker (CHW) Supervisor is responsible for the supervision, coordination, and day-to-day
management of Community Health Workers (CHWs) who support patients, families and community members in
addressing social determinants of health, navigating health and social service systems, coordinating care, supporting
chronic disease self-management and promoting health and wellness. The CHW Supervisor provides leadership, coaching,
training, and operational oversight to ensure high quality, patient-centered, community-based services aligned with
BCHN's mission and program goals. The CHW Supervisor collaborates with clinical teams, care management staff,
behavioral health providers, community organizations, and external partners to improve patient engagement, care
coordination, health equity, and access to essential resources.

Requirements
- Supervise, coach, and evaluate Community Health Workers and related support staff.
- Conduct regular staff meetings, individual supervision sessions, and performance evaluations; provide timely feedback and
- support performance improvement as needed.
- Develop staffing schedules, assign caseloads and outreach responsibilities, and ensure appropriate coverage and workload
- distribution.
- Monitor CHW productivity, caseloads, service quality, documentation, and program performance metrics; use data to
- identify opportunities for improvement.
- Assess CHW training needs and coordinate ongoing training, professional development, and competency-building
- opportunities for CHW staff.
- Promote a culture of teamwork, accountability, patient centered care, and continuous quality improvement.
- Oversee outreach and patient engagement activities designed to connect patients and community members to primary care,
- preventive services, and community resources.
- Ensure CHWs identify and address barriers to care and social needs, including housing, food insecurity, transportation,
- benefits, employment, education, and other health-related social needs.
- Develop and maintain effective professional relationships with community-based organizations, social service agencies,
- schools, faith-based organizations, public health entities, and other community partners.
- Maintain knowledge of available community resources and strengthen referral pathways for patients and families,
- including closed-loop referral processes where applicable.
- Represent the organization at community meetings, events, and collaborative initiatives.
- Support and oversee health education, chronic disease self-management, and community engagement activities within the
- CHW scope of practice.
- Ensure timely, accurate, and complete documentation of patient encounters, screenings, referrals, interventions and
- outcomes in the electronic health record (EHR) and other designated systems.
- Prepare and review reports related to CHW productivity, program performance, outreach activities, social needs
- interventions, referrals, and outcomes.
- Track and monitor referrals, resource utilization, patient engagement, and outcomes; identify gaps and follow up on unresolved referrals as appropriate.
- Ensure CHW documentation and workflows comply with organizational policies, program requirements, and applicable regulatory and payer standards.
- Maintain confidentiality and ensure compliance with HIPAA and organizational privacy and security requirements.
- Support compliance with applicable HRSA Health Center Program requirements, FQHC requirements, NYS regulations, grant requirements, and other federal, state, and local requirements relevant to CHW programs.
- Support data collection, validation, and reporting for the Uniform Data System (UDS), quality improvement initiatives, value-based care programs, and grant-funded programs, as applicable.
- Assist with audit readiness, site visits, program reviews, and performance improvement activities related to community health and population health programs.
- Maintain knowledge of and compliance with applicable FQHC, HRSA, Article 28, HIPAA, OSHA, infection control, patient privacy, and federal, state, and local regulatory requirements.
- Supports audit readiness, survey preparation, documentation standards, and organizational policies as applicable to the role.
- Perform other duties, as assigned based on organizational needs, including support for new site openings, mobile health operations, emergency preparedness, community outreach and special projects.
