Local Pay: $32.00/hour (W2)

Shift Details:
- Monday-Friday 8:00am-4:30pm
Duties & Responsibilities:
- Assessing individuals, developing individual care plans and determining necessary resources.
- Advocating for and assisting individuals and families in accessing and obtaining services.
- Contacting service providers and attending interdisciplinary conferences to determine appropriate placement and referrals.
- Monitoring individual rehabilitation goals and progress.
- Providing crisis intervention services.
Minimum Qualifications
- LMSW License is required
- Background check requirements:
Selected candidates must complete and be cleared through the required finger printing and Statewide Central Register. - Additional certifications may be required.
- Copy of current, completed Primary Source Verification (PSV) from the appropriate licensing board or verification agency, if required.
- Copy of all current required certifications and compliance documents (e.g., BLS, N95 fit test, immunization records, TB test, or other client-specific requirements).
