Social Work/Care Companion
Join Carease Health in Flushing, NY and make a meaningful impact on the lives of older adults! As a Social Work/Care Companion
, you will provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities.
At Carease Health, we prioritize proactive measures for enhanced physical health and well-being through the incorporation of Remote Patient Monitoring (RPM), Chronic Care Management (CCM) and Collaborative Care Management (CoCM) into our comprehensive preventative care program. As a part of our Collaborative Care team, you will work to minimize hospitalizations, elevate patient care standards, and boost CMS ratings.
Key Responsibilities- Visit designated facilities in accordance with the schedule set with your supervisor
- Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs
- Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns
- Document visit notes and maintain detailed records of interactions and interventions after each visit
- Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP
- Associates, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health–related field
- Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings
- Strong communication, documentation, and relationship-building skills
- 1 year of experience
- Meaningful, resident-focused work in a supportive team environment
- Regular supervision and collaboration with psychiatric providers
