We are currently hiring for a Patient Care Coordinator position with one of our clients in New York, NY. This is an excellent opportunity for healthcare professionals interested in patient services, healthcare coordination, clinical operations, and patient support

The ideal candidate will have strong communication and organizational skills, a patient-focused approach, attention to detail, and the ability to coordinate effectively with patients, healthcare providers, and administrative teams.

Key Responsibilities- Coordinate patient appointments, referrals, follow-ups, and other healthcare services.
- Serve as a point of contact for patients and assist them throughout their care journey.
- Communicate with patients regarding appointments, procedures, care instructions, and follow-up requirements.
- Coordinate with physicians, nurses, clinical staff, and other healthcare professionals to support patient care.
- Maintain accurate and up-to-date patient records and documentation.
- Assist with insurance verification, authorizations, referrals, and other administrative processes.
- Respond to patient inquiries and help resolve scheduling or service-related concerns.
- Follow established privacy, confidentiality, and healthcare compliance requirements.
- Assist with coordinating referrals between departments, specialists, and external healthcare providers.
- Monitor appointment schedules and follow up on missed appointments or required services.
- Enter and maintain patient information in electronic health record (EHR) and other healthcare systems.
- Support patient intake, registration, and documentation processes.
- Prepare reports and maintain records related to patient services and operational activities.
- Identify patient service issues and elevate complex concerns to appropriate clinical or administrative staff.
- Contribute to process improvement initiatives that enhance patient experience and care coordination.
- Bachelor's or Master's degree in Healthcare Administration, Health Administration, Health Informatics, Public Health, Nursing, Health Sciences, Social Work, Business Administration, or a related field
. - Strong communication and interpersonal skills.
- Excellent organizational and time-management abilities.
- Strong attention to detail and accuracy.
- Ability to communicate effectively with patients from diverse backgrounds.
- Ability to handle confidential and sensitive information professionally.
- Basic understanding of healthcare operations and patient care processes.
- Proficiency with Microsoft Office, particularly Word and Excel.
- Ability to work independently and collaboratively in a healthcare environment.
- Internship, clinical rotation, practicum, or work experience in a hospital, clinic, physician practice, healthcare organization, or medical office
. - Familiarity with Electronic Health Records (EHR/EMR) systems.
- Knowledge of healthcare scheduling, referrals, insurance verification, or prior authorization processes.
- Understanding of HIPAA and patient privacy requirements.
- Familiarity with medical terminology.
- Experience with Epic, Cerner, or other healthcare information systems is a plus.
- Bilingual or multilingual communication skills are a plus.
- Experience with patient intake, registration, care coordination, or healthcare customer service is beneficial.
