Bring your talents to New York-Presbyterian and help drive the success of our Center for Community Health Navigation’s Health Promotion Program.

The Health Promotion Advocate (HPA) supports New York-Presbyterian's emphasis on health equity by screening patient for social needs and, when needs are identified, connecting patients to resources to address their needs. HPAs also empower patients to understand how to enroll onto and to navigate our patient portal so that they can access their records, schedule appointments, and communicate with providers.

The Health Promotion Advocate administers social determinant of health screening, connects patients to resources, and empowers patients to navigate the patient portal. Advocates are responsible for carrying out the duties for the assigned program and site.

• Community-based and/or healthcare experience
• Bachelor's degree
• Fluent in Spanish

• High School diploma or GED
• Strong written and verbal communication skills
• Microsoft Word and Excel experience
