MUSC Community Physicians (MCP) seeks a QMHP / Social Worker to coordinate discharge planning and ensure a smooth transition with the health care team. The role includes assessing patients and caregivers during hospitalization and outpatient follow-up to mitigate post-discharge risks.
Requires a Master’s degree in Social Work (MSW) and SC licensure. Evening shift at Lexington County Detention Center with an eligible $1,000 sign-on bonus and comprehensive benefits.
