About the Role:
As an Occupational Therapist (OT) in our Skilled Nursing Facility, you will be responsible for designing and implementing customized therapy plans to help patients achieve their maximum potential. You will work closely with our interdisciplinary team of healthcare professionals to provide comprehensive care that addresses the physical, emotional, and social needs of our patients. Your expertise will enable patients to regain independence, build confidence, and maintain their autonomy.

Responsibilities:

• Assess patients' occupational therapy needs and develop individualized treatment plans
• Implement and modify therapy plans to ensure safe and effective patient care
• Collaborate with other healthcare professionals to ensure comprehensive care
• Educate patients and their families on proper techniques and strategies for self-care
• Maintain accurate and timely documentation of patient progress and care
• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

Requirements:

• Occupational Therapist (OT) certification and licensure (New York state)
• Bachelor's/Master's degree in Occupational Therapy or a related field
