We have immediate opportunities for therapists to provide one-on-one skilled therapy services to patients in their homes throughout Palm Beach County
, including West Palm Beach, Boca Raton, Delray Beach, Boynton Beach, Lake Worth, Wellington, Royal Palm Beach, Greenacres, Palm Beach Gardens, Jupiter
, and surrounding communities.

• Perform comprehensive patient evaluations

• Develop individualized treatment plans

• Provide skilled physical or occupational therapy services in the home

• Educate patients and caregivers on treatment plans and home exercise programs

• Accurately document patient progress and treatment outcomes

• Collaborate with physicians, nurses, and the interdisciplinary care team to ensure exceptional patient care

• Active Florida Physical Therapist (PT) or Occupational Therapist (OT) license

• Home health experience preferred

• Strong clinical assessment and documentation skills

• Excellent communication and patient education abilities
