The Role ******:
**** Interviews and screens individuals for supportive counseling;
* Plans, implements and evaluates an individualized treatment plan for each patient;
* Monitors patient progress and makes recommendations for further treatment if necessary;
* Provides individual and group counseling services;
* Maintains patient records and reports;
* Collaborates with supervisors and clinic staff regarding patient progress and patient services;
* May conduct education and information sessions, workshops, and training programs for individuals with substance use disorders and members of their community

*** Applicants must either have a current certification from NYS Office of Alcohol and Substance Abuse Services (OASAS) as a Credentialed Alcoholism and Substance Abuse Counselor (CASAC),

* *** Possession of the requirements necessary to be designated as a Qualified Health Professional (QHP) with one year of experience or satisfactory completion of a training program in the treatment of substance use disorders.
