Summit Home Care is a NYS licensed home care agency that services all 5 Boroughs of NYC and Long Island. We currently have adult and pediatric patients in Sufflok that need exceptional,compassionate, and reliable Home Health Aides!

• Daily personal care (bathing, grooming, oral care, dressing, etc.)

• Assist with the daily living activities per the Plan of Care

• Monitor patients and report on their condition

• Help with patient meals and feeding, as well as basic housekeeping functions

• Accompany the patient to doctor's visits, shopping, and other errands

• Must have New York State Home Health Aide Certificate

• Government Issued , Social Security Card & Work Authorization, if needed

• 2 Reference Letters

• Recent Physical

• TB Screening (Quantiferon preferred)

• Rubella & Rubeola titer lab work
