Freedom Care LLC is seeking a Case Manager in upstate NY to support the Health Home Care Management Program. You will outreach to potentially eligible patients, obtain consents, and enroll them in the program, then work with the care team to assess needs and coordinate care per the care plan.
Responsibilities include conducting assessments, developing care plans, coordinating services, and ensuring documentation meets Health Homes standards while maintaining confidentiality.
