Extended Home Care is seeking an experienced Occupational Therapist to conduct home visits for IDD patients across Bronx, Manhattan, Queens, and NY. The OT will implement therapy plans, assess functional status, and collaborate with physicians to develop and revise the plan of care.
Responsibilities include initial assessments, periodic re-assessments, documentation, and patient/family instruction on safety and mobility techniques.
