Social Worker- Bilingual; Spanish​/English

We are currently looking for a dedicated Social Worker to join our team and make a meaningful impact by offering emotional support, advocating for residents' needs, and guiding families through the complexities of both short-term and long-term care.

• Assess the social, emotional, and environmental needs of residents and their families.
• Develop and implement individualized care plans in collaboration with the healthcare team.
• Provide emotional support, counseling, and crisis intervention to residents and families.
• Assist new residents with the transition process and connect families with available resources.
• Advocate for residents’ rights, ensuring they are treated with dignity and respect.
• Help residents access community resources and support services as needed.
• Maintain thorough and accurate documentation of social work interventions and resident progress.
• Participate in care plan meetings and collaborate with the multidisciplinary team.
• Ensure compliance with state and federal regulations related to social work services.

• Bilingual in English and Spanish
• Previous experience in long-term care or healthcare settings is preferred but not required.
• Strong communication, interpersonal, and organizational skills.
• Compassion, patience, and a genuine passion for helping others.
• Ability to work independently and collaboratively as part of a team.
