Overview

Case Planning Program Manager needed for ICAN in Utica, NY. Pay rate is $64,000 - $69,000 per year. Integrated Community Alternatives Network (ICAN), a Community-based Wraparound provider of Social and Mental Health services with an emphasis on treating the client in a family context, is currently seeking innovative and experienced applicants to fill a Full-Time Case Planning Program Manager position.

The Case Planning Program Manager is responsible for supporting the development, implementation, and strategic management of ICANs Oneida County Case Planning program, in partnership with Oneida County Department of Family and Community Services (OCDFC). This position ensures the delivery of high-quality, strength-based, and family-centered preventive services for families referred by OCDFC. The Program Manager oversees quality assurance practices and maintains oversight of the Case Planning team, including program direction and administrative supervision of staff while adhering to and promoting the philosophy and mission of the company.

Responsibilities
  • Provide program leadership to ensure delivery of high-quality, family-centered, strengths-based preventive services that align with agency mission, values, and contractual requirements.
  • Participate in recruitment, interviewing, and onboarding of Supervisors and Case Planners, as well as the orientation and training of new staff, ensuring they understand agency policies, best practices, and program requirements.
  • Supervise Supervisors and Case Planners, offering support and guidance on case management, crisis intervention, and emergency response to ensure adherence to professional and ethical standards.
  • Serve as liaison between ICAN and Oneida County.
  • Act as main point of contact for all incoming referrals.
  • Routinely review and monitor program/staff documentation, service plans, and reports to ensure compliance with agency standards, NYS regulations, and OCDFC requirements.
  • Develop and maintain quality assurance processes to track outcomes, ensure accountability, and promote continuous program improvement.
  • Oversee training and professional development opportunities for program staff; lead implementation of case planning best practices across the program.
  • Partner with the Director, Supervisors, and other agency/program staff to assess case planning needs, identify program gaps, and recommend strategies to achieve outcomes.
  • Compile and prepare routine data/outcome reports for agency and/or contractor.
  • Represent the program at community meetings, advisory boards, and collaborative committees to build strong partner relationships.
  • Foster positive working relationships with community stakeholders, county partners, and cross-program colleagues to strengthen service delivery. Partner with agency’s QI and compliance departments to ensure all incidents, grievances, and reports are documented and reviewed according to agency policies, regulatory requirements, and best practices.
  • Provide support to families directly as needed through phone calls or in-person meetings to ensure program responsiveness and continuity of care.
  • Travel as required, primarily within Oneida County, with occasional travel within New York State.
  • Other duties may be assigned, on an as needed basis.
Education/Experience
  • Bachelor's degree (B.A.) from an accredited four-year college or university, a major in social work or a related field is preferred.
  • A minimum of two years of experience providing service coordination for community-based services to individuals and families, preferably through the child welfare system.
  • A valid NYS Driver's License is required.
  • A minimum of 2+ years supervisory experience is required.
