A premiere rehabilitation and skilled nursing facility in Staten Island, NY is seeking an experienced Director of Social Work (DSW) to lead our Social Services Department. • Direct, establish, and plan overall department goals, policies, and procedures.

• Oversee resident assessments, personalized care planning, and transition/adjustment support.

• Lead, mentor, and supervise Social Services Department staff.

• Manage discharge planning initiatives starting from resident admission.

• Perform occasional community outreach to foster local partnerships.

• Apply extensive clinical judgment, creativity, and industry best practices to achieve quality-of-care goals.

• Education/Licensure: Licensed Social Worker (LSW) or Master of Social Work (MSW required.

• Experience: 2+ years of supervisory or management experience in a Long-Term Care (LTC) setting.

• Knowledge: Deep understanding of applicable State and Federal healthcare guidelines and regulations.

• Skills: Exceptional interpersonal, communication, and leadership capabilities.
