• Join our team as a Nurse Practitioner in a skilled nursing facility located in Brooklyn, NY.
• This full-time position requires providing comprehensive medical care, conducting patient assessments, and ensuring accurate documentation.
• The role involves working 40 hours per week, five days a week, utilizing electronic documentation and general office software.

• Provide medical care in a skilled nursing facility setting.
• Conduct regular patient assessments and manage chronic and acute conditions.
• Ensure accurate documentation of patient information.
• Collaborate with interdisciplinary staff to develop care plans.
• Monitor and respond to acute changes in patient conditions.

• Perform in-person clinical visits regularly and as needed.
• Review and document patient medical histories.
• Conduct clinical assessments of long-term care residents.
• Ensure accurate documentation of clinical encounters for billing and compliance.

• Current and valid license and registration as a Registered Professional Nurse and Nurse Practitioner.
• 3 years of clinical nursing practice in home care, skilled nursing facility, or hospital.
• 3500 hours as a Nurse Practitioner with NPI # and ability to bill Medicaid and Medicare.
• Experience with electronic documentation and general office software.
