Job Requirements

• Patient Assessment: Conduct nursing assessments by collecting and documenting medical histories, performing physical examinations, and continuously monitoring patient conditions.
• Care Planning and Implementation: Develop and execute individualized patient care plans in collaboration with physicians and interdisciplinary teams. Adjust care based on ongoing evaluation and emerging patient needs.
• Medication Administration & Procedures: Administer prescribed medications (oral, IV, or IM), vaccinations, and other treatments as ordered. Perform or assist with clinical procedures such as dressing changes, wound care, blood draws, and EKGs.
• Patient & Family Education: Provide education to patients and families on medical conditions, medication management, treatment plans, self-care, and preventive health strategies
• Care Coordination: Coordinate referrals, follow-up appointments, and continuity of care across different services or settings. Work with other care providers and support staff to ensure seamless patient experiences.
• Documentation: Maintain accurate and timely patient records, including assessments, interventions, care plans, and patient progress. Ensure compliance with legal and hospital/clinic guidelines.
• Collaboration & Communication: Serve as a liaison among providers, patients, families, and pharmacies. Participate in team meetings and communicate effectively with all healthcare partners.
• Telephone Triage: Provide guidance and assess urgent needs by phone, triaging symptoms and advising patients appropriately.
• Patient Advocacy & Safety: Advocate for patient well-being, uphold infection control policies, promote patient safety, and respond to emergencies or acute clinical changes.

Additional Duties (May Vary by Setting)

• Support in Specialized Care: May assist with perioperative care, chronic disease management, or rehabilitation services, depending on the clinic or hospital unit.
• Leadership & Mentoring: Guide or orient new nurses and support staff, and participate in ongoing professional development and continuing education.
• Quality Improvement: Engage in quality assurance, process improvement, and evidence-based practice initiatives within the clinical setting.

Required Skills & Qualifications

• Current state RN licensure and CPR certification (additional certifications may be required)
• Strong clinical assessment, critical thinking, and communication skills.
• Proficiency in nursing procedures, use of diagnostic equipment, and electronic health record (EHR) documentation.
• Compassion, adaptability, strong organizational abilities, and a commitment to professional ethics.
