Job Summary
Licensed Practical Nurse (LPN) – SNF
Shift: Variable – Days;
Variable – Evenings/Days
Hours:
8-hour shifts / 40 hours per week guaranteed
Setting:
Skilled Nursing Facility (SNF)
Assignment Length: 13 weeks
- Deliver direct patient care in accordance with the plan of care under the supervision of an RN or physician
- Administer medications and treatments as prescribed
- Monitor and document patient conditions and report changes to the healthcare team
- Provide support with activities of daily living (ADLs)
- Collaborate with interdisciplinary team members to ensure quality patient care
- Active LPN license in state of assignment
- Minimum of 1 year of recent LPN experience, preferably in a SNF or long‑term care setting
- BLS certification (AHA preferred)
- Strong communication and organizational skills
- Ability to work flexible shifts as scheduled
