Licensed Practical Nurse, Home Health

Overview

Licensed Practical Nurse, Home Health - Center Well Home Health. Provide personalized, one-on-one nursing care in patients' homes, working with a dedicated team of physicians and clinicians to develop and manage care plans that support recovery and independence.

• Provide professional nursing services under the direction of an RN in compliance with the state's Nurse Practice Act, licensure/certification requirements, healthcare standards, laws and Center Well Home Health policies.
• Deliver skilled nursing interventions for illness, rehabilitative needs and preventative care; apply a holistic approach to plan of care with teaching and training activities.
• Use established standards of clinical practice to facilitate problem resolution and achieve individualized patient goals and outcomes.
• Coordinate with the RN Supervisor on changes to the Plan of Treatment; accept verbal orders as permitted by state law and communicate these orders appropriately.
• Implement the nursing plan of care with a systematic, goal/outcome-driven approach.
• Maintain timely communication with patients, physicians, and other health care team members regarding needs and status changes; participate in care coordination and discharge planning as appropriate.
• Maintain professional confidentiality and share information only when legally appropriate and in the best interest of the patient.
• Ensure availability of supplies, materials and equipment needed to safely implement the plan of care.
• Prepare, submit and maintain documentation as required by the Company; visit/shift notes documented on day of service.

• Graduate of an accredited Licensed Practical Nursing Program or School of Vocational Nursing.
• Current nursing license in the practicing state.
• Valid driver’s license, auto insurance and reliable transportation.
• Current CPR certification.
• Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice.

• Base pay range: $44,600.00/yr - $61,400.00/yr
• Pay per visit/unit: $28.00 - $40.00
• Scheduled weekly hours: 40
