Location: Town of Vestal

Position Details

  • Schedule:

    Typically 8:00 AM - 4:30 PM, Monday - Friday, with flexibility to create your own daily schedule based on patient load.
  • On-Call Rotation:
    Typically once a week.
  • Weekend Rotation:
    Typically one weekend a month.

Who We Are

At Guthrie Home Care in Vestal, NY, we are dedicated to providing exceptional home health services to support patients recovering from hospitalization, illness, or injury. Our incredible team of Case Managers are critical thinkers and compassionate caregivers delivering the highest standard of care directly to patients in their homes.

We prefer one year of experience in Med/Surg, ER, or ICU to ensure our patients receive the best care possible.

Description

Provides direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization.

Education / License

Diploma from an accredited school/college of nursing. Required professional licensure at time of hire.

Responsibilities
  • Implements and monitors patient care plans; monitors, records, and communicates patient condition as appropriate.
  • Performs direct participant care, including acute triage, blood draws, wound care, etc.
  • Participates in the development, implementation, and evaluation of the Interdisciplinary Team (IDT) plan of care; serves as primary coordinator of all disciplines for well-coordinated patient care.
  • Carries out physician and nursing orders; attends team meetings and care plan meetings.
  • Assesses and coordinates patients' discharge planning needs with members of the healthcare team; participates, develops, and provides education to participants, caregivers, and associates.
  • Participates in after hours / weekend on-call rotation, as needed; may require travel to participant places of residence, long term care, and assisted living facilities.

The payrate for this position is from $81,120 - $.
