We are currently expanding our geriatric behavioral health team and are seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP) who shares our commitment to exceptional patient-centered care. Key Responsibilities- Perform comprehensive psychiatric assessments and develop individualized treatment plans.
- Prescribe, manage, and adjust psychotropic medications in collaboration with attending providers.
- Provide training and educational sessions for facility staff on behavioral health best practices.
- Work closely with nursing, therapy, and social work teams to ensure coordinated, holistic care.
- Maintain accurate, compliant, and timely documentation in the electronic health record (EHR).
- Participate in interdisciplinary meetings, case reviews, and quality improvement initiatives.
- Active New York State Nurse Practitioner license and current PMHNP certification.
- At least one year of psychiatric or behavioral health experience is preferred (new graduates encouraged to apply).
- Proficient understanding of psychopharmacology and evidence-based interventions.
- Strong clinical judgment, diagnostic, and communication skills.
