Overview

The Registered Nurse Case Manager is responsible for providing direct patient care, coordination of overall home care services and documentation of care for agency clients in accordance with Quality Standards and under the guidance of a Clinical Supervisor. This role will primarily support Oswego County. We have a fleet vehicle program available that supports this clinical role.

Home visits are conducted Monday-Friday, 8:00 a.m.–4:30 p.m., with one weekend per month required. There is no on-call required.

Responsibilities
  • Ensure quality and safe delivery of home health services including case management of client caseloads, developing a plan of care based on client assessment and OASIS evaluation, and ensuring that all documentation meets quality standards.
  • Complete all OASIS evaluations; ensure nursing care and interactions are documented clearly and promptly in accordance with organizational standards.
  • Coordinate LPNs; provide guidance and education involving client care to LPNs; work with the Clinical Supervisor and Patient Care Coordinator to efficiently manage patients using a team approach.
  • Supervise paraprofessional staff related to patient care; supervise home health aides and communicate relevant information to the home health aide department.
  • Comply with Nascentia Health policies and quality initiatives; maintain patient/employee confidentiality; demonstrate infection control and compliance with applicable rules and regulations.
  • Adhere to organizational quality improvement standards and maintain and update clinical knowledge on an ongoing basis.
Qualifications
  • Current New York State registered professional nurse license (degree from an accredited school of nursing). BSN a plus.
  • Current and valid driver’s license and auto insurance, and a vehicle for work (fleet program available).
  • Self-directed, organized, and able to work independently in a fast-paced environment.
  • Experience with Hospice, End of Life, and/or Palliative Care preferred.
  • Proficient computer skills, including electronic medical records software; ability to learn new software.
  • Exceptional oral, written and verbal communication skills.
Physical Requirements
  • Ability to sit or stand at a desk 90% of the day
  • Physical tasks including pushing/pulling up to 50 lbs on average (max 150 lbs); lifting/carrying up to 25 lbs on average (max 40 lbs); patient transfers, bending, standing, kneeling, and walking in various conditions, with ability to ascend/descend stairs
  • Ability to drive in all weather conditions
  • Visual/hearing ability sufficient to comprehend written or verbal communication
  • Ability to use a telephone and communicate over the phone
  • Frequently uses a laptop computer to enter orders and obtain information
  • Bloodborne Pathogens Exposure Category III – employees who rarely have contact with blood and body fluids
