Become part of a caring community and help put health first. As a Home Health Registered Nurse you will provide personalized, one‑on‑one care to patients who are recovering from injury, illness, surgery, or hospitalization in the comfort of their homes.
Pay Range- Base pay: $70,500 – $96,900 per year
- Pay per visit/unit: $45.00 – $63.00
- Provide admission, case management, and follow‑up skilled nursing visits for home‑health patients.
- Administer ongoing care and case management, and provide necessary follow‑up as directed by the Clinical Manager.
- Confer with physicians to develop initial treatment plans and revise plans in consultation with physicians based on ongoing assessments.
- Coordinate appropriate care with physical therapists, occupational therapists, home‑health aides, and external providers.
- Report patient care, condition, and progress to physicians and the Clinical Manager continuously.
- Implement patient care plans in conjunction with patients and families to assist them in achieving optimal outcomes.
- Supervise and coordinate the work of Home‑Health Aides and Personal Care Workers, provide written instructions, and monitor documentation completion.
- Discharge patients after consultation with the physician and Clinical Manager, preparing all required clinical documentation.
- Prepare comprehensive medical documentation, including case conferences, medication changes, re‑certifications, and care‑plan updates.
- Diploma, Associate, or Bachelor’s Degree in Nursing.
- Minimum one year of nursing experience preferred.
- Strong experience in medical‑surgical, ICU, ER, and acute care settings.
- Home‑health experience is a plus.
- Current and unrestricted Registered Nurse license.
- Current CPR certification.
- Strong organizational and communication skills.
- Valid driver’s license, auto insurance, and reliable transportation.
