Day Shift (Monday – Friday 8:00 a.m. – 4:30 p.m.)

Work as part of our Hospice Homecare Team caring for patients and families residing in the Rensselaer/Albany County region.

• Coordinate with an interdisciplinary team to develop a comprehensive plan of care and provide added support to patients and families in their homes
• Work closely with aides, LPNs, medical directors, NPs, social workers, chaplains, complementary therapists, and volunteers

• A current license to practice as a Registered Nurse in the State of New York
• Associate’s Degree in Nursing; BSN preferred
• Minimum 1 year of RN experience, preferably in Hospice/Home Care or acute care
• Valid driver’s license and reliable vehicle
• Strong verbal and written communication skills
• High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers
• Understanding of hospice principles preferred
• Training provided in end‑of‑life care

Training in end‑of‑life care, thorough orientation, preceptors and a supportive work environment.

$35.00 - $47.75 per hour (based on experience, skills, and education). Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates; pay may vary within the stated range based on location.
