Position: Hematology Nurse Navigator (Grant Funded)
Location: Valhalla

Overview

Hematology Nurse Navigator (Grant Funded) – Grant funded position. The Hematology Nurse Navigator will assist and coordinate care of Hematology patients, with special emphasis on the transition of adolescent and young adult subjects with sickle cell disease to adult care. This position directly reports to the Principal Investigator.

Responsibilities
  • Assist in the care and coordination of Hematology patients.
  • Support the transition of adolescent and young adult patients with sickle cell disease to adult care.
  • Identify unit system problems and take steps to rectify them, in cooperation with other health team members.
  • Exhibit courteous and respectful behaviors and attitudes toward all members of the health care team.
  • Complete all documentation accurately.
  • Maintain a professional and positive attitude and provide consistent, exceptional customer service to internal and external personnel, patients and visitors.
  • Provide on-call support for nights, weekends, and/or holidays.
  • Collaborate with the work team to identify patient and family needs.
  • Maintain current knowledge in the area of responsibility.
  • Maintain licensure, certification or registration as required.
  • Adapt to new and unusual situations or changes without affecting performance.
  • Perform job duties within the time standards as determined by the division.
  • Regularly report to work as scheduled and return from meal breaks within allotted time frames.
  • Maintain a professional appearance and neatness as outlined by APS guidelines.
  • Act as a collaborative team member, supporting decisions, accepting change, managing conflict effectively, and valuing others’ contributions.
  • Demonstrate professionalism through courteous, prompt patient, staff, and vendor encounters and uphold the organization’s values and commitment to quality patient care.
  • Support and facilitate grant-funded research entitled “AYA SCD Transition and Nurse Navigator for Implementing Home Health Model.”
  • Draft required Quarterly Reports to NYS as finalized by the Principal Investigator of the grant.
  • Conduct and perform required research relevant to this grant.
Qualifications/Requirements
  • Experience:

    2 years of prior hematology, oncology, stem cell transplantation, and/or adolescent and young adult general medicine experience, required.
  • Education:

    Graduate from an accredited school of nursing, required;
    Bachelor of Science in Nursing (BSN) degree, preferred.
  • Licenses /

    Certifications:

    Active NYS Registered Nursing License, required; BLS, required.
  • Other:
    Familiar with FDA-approved automated CBC machines; computer, telephone system, copy machine, fax, paper shredder, calculator, and various standard office equipment.
