Visit Nurse - Home Care (Part Time)
Hudson Valley Hospice

• Maintain accurate records of patient/family care services.
• Assess needs of terminally ill patients and contribute to the Interdisciplinary Team Plan of Care.
• Collaborate with team members to meet patient needs.
• Revise Plan of Care—including medications, projected visits, advanced directive status, functional limitations, and treatments—as needed.
• Provide 24‑hour continuous skilled nursing care for patients/families in their homes as required.
• Assist case manager/TL/DPS in managing patient case loads and responding to patient/family needs in accordance with the Plan of Care.
• Maintain working knowledge of the CADD‑Prizm pump.

• Valid New York State RN license.
• Graduate of an accredited nursing program.
• Minimum of two (2) years paid experience in nursing; experience in oncology, home care or hospice preferred.
• Experience caring for terminally ill patients and a basic understanding and commitment to the hospice philosophy preferred.
• Experience working in a comprehensive interdisciplinary team approach to care preferred.

Variable shifts available – open to flexible hours and schedules. 20–32 hours/week options.

• Moderate exposure to highly contaminated agents.
• Regular travel by car in all weather conditions.
• Work in patient homes, hospitals and nursing homes.
• Potential direct exposure to blood or body fluids.
• Ability to carry up to twenty‑five pounds of equipment and/or supplies into a patient’s home and transfer/position an average adult for care.

Pay: $38–43 per hour
