• Perform on‑site screening clinical assessments to determine appropriate patient treatment plans for residents at client facilities.
• Function independently to perform age‑appropriate history and physical exams for complex acute, critical, and chronically ill perioperative patients.
• Order and interpret diagnostic and therapeutic tests relative to patient‑specific needs.
• Assist with provision of care in accordance with facility, state, and federal regulations.
• Monitor the effectiveness of interventions.
• Maintain relationships with all appropriate contacts in facilities.
• Educate family members, clients, and prospective accounts regarding services and programs.
• Assist with the development and maintenance of physician relations, as assigned.
• Complete weekly, monthly, and other reports in a timely and comprehensive manner.
• Other duties as assigned by the Medical Director.

• Current Nurse Practitioner / Physician Assistant license.
• Degree from an accredited Nurse Practitioner / Physician Assistant program.
• ACLS & BLS certifications.
• Current infection control certification.
• Strong clinical collaboration with physicians.
• Proficient computer skills.
• PACS and EMR experience preferred.
• Committed to professional growth and learning.
• Clean driving record.
• Personal vehicle ownership required.
• Local travel required.
• Not listed on the OIG exclusion list.

• Vascular care: venous insufficiency, peripheral arterial disease.
• Image guided injectables.
• Advanced geriatric care.
• Experience in skilled nursing facilities, assisted living facilities.
