Full Time Remote (must reside within the New York Tri‑State Area - NY, NJ, or CT)

Village Care is hiring a Full Time RN
- Care Manager to provide remote care and support to our members in the greater NYC area. You will showcase your nursing expertise while managing the care needs of our diverse patient population. The role offers a competitive salary ranging from $95,000 to $105,000 per year and a comprehensive benefits package.

Competitive salary: $95,000 – $105,000 per year.

• Play a crucial role in all aspects of care management, including care planning, coordination, and health assessments.
• Monitor the quality and effectiveness of services; track progress toward individual goals and ensure the highest level of care for patients.
• Oversee transitions for patients and facilitate smooth discharge planning from hospitals or nursing homes.
• Identify health, environmental, and psychosocial risks; recommend and implement interventions in collaboration with the interdisciplinary care team.

• Valid NYS RN License.
• Minimum of 3 years of relevant experience in managed care, home care, or community settings.
• Associate’s degree or higher required.
• Exceptional communication skills, strong problem‑solving abilities, and a passionate attitude.
• Proficient in Korean, Bengali, Cantonese, Mandarin, Spanish, Russian, or Creole language.
• Ability to navigate and utilize various software and tools relevant to care management.
