Responsible for ensuring optimal continuity of care for newborn patients and their families through support, coordination, and education of care, fostering collaboration among physicians and the broader interdisciplinary care team participating in clinical activities. This position provides health care including clinical assessment and follow-up.

• Provide patient coordination and maintain an overview of patients throughout the continuum of care. Serve as a team leader in conducting interdisciplinary patient care.
• Deliver direct patient care: assess clinical status, triage patients to appropriate physicians or providers, troubleshoot care issues, and communicate unresolved issues with staff.
• Coordinate care for patients with acute illnesses, complete treatments for care episodes, manage diagnostic testing, ensure timely follow-up, and actively communicate with referring physicians.
• Ensure timely follow-up strategies and consult with nursing staff or clinical care team members for patients with complex health problems requiring specialized care.
• Manage quality metrics together with hospital initiatives and collect/analyze data from internal and external benchmarking, research findings, and outcome data to improve services and patient outcomes.
• Maintain a working knowledge of payer requirements and services accessible to patients participating in common plans.

• Bachelor’s Degree in Nursing

Approximately 3-5 Years of Related Experience

Must be articulate and possess a professional demeanor.

• Current NY State Registered Nurse License & Registration
• BLS Certification (American Heart Association)
• Infection Control Certificate
