Location: New York

Pride Health is hiring a Nurse Practitioner in Steuben County, NY!

Schedule: Monday-Friday (8am-5pm)

Contract: 1 year, Potential temp to hire

Pay Rate: $65-70/hour

Specific Duties :

Provide specialized care to high-risk health plan members
Work as part of an interdisciplinary team
Manage acute and chronic medical and behavioral conditions
Communicate and collaborate with the care team
Facilitate advanced illness and care planning discussions
Offer counseling and education to patients and caregivers
No on-call
No holidays required

Must have qualifications/experience:

NP Certified in one of the following (FNP, ANP, GNP)
Access to transportation
1+ year of experience (LTC, SNF, Geriatric preferred)
