We are currently seeking Licensed Practical Nurses (LPNs) to provide high-quality, one-on-one care to patients in their homes throughout Dutchess County on 12-hour shifts. Apply today to join our growing community and make a real difference in patients’ lives.

• Consistent, one-on-one patient assignments

• Provide skilled nursing care in the home under the direction of an RN
• Administer medications and treatments as prescribed
• Perform vital signs monitoring and patient observations
• Assist with wound care, Peg Tub feedings and other skilled nursing tasks
• Document patient care accurately and in a timely manner
• Communicate changes in patient condition to supervising RN and care team
• Educate patients and families on basic health management and care routines
• Ensure patient safety and deliver compassionate, high-quality care in the home setting

• Current NYS Licensed Practical Nurse (LPN) license in good standing
• Meets NYS health requirements (recent physical, immunizations)
• Minimum 1 year of nursing experience (home care experience preferred but not required)
• CPR certification (required)
• Strong communication, organizational, and interpersonal skills
• Valid driver’s license or reliable transportation
