New Vanderbilt Rehabilitation and Care Center is seeking a dedicated Wound Care Nurse (RN) to join the team. As a wound care nurse, you will be responsible for assessing, planning, implementing, and evaluating wound care interventions for residents with acute or chronic wounds. You will collaborate with the interdisciplinary team to promote a healing environment and ensure the provision of high-quality care. Your expertise in wound assessment, treatment, and prevention will be critical in promoting optimal resident outcomes.
Responsibilities- Perform comprehensive assessments of residents with wounds, including wound measurement, staging, and identification of underlying causes.
- Develop individualized care plans for residents with wounds, considering their medical history, co-morbidities, and treatment goals.
- Implement evidence-based wound care interventions, including wound cleansing, dressing selection, debridement, and application of topical medications.
- Monitor and evaluate the progress of wound healing, documenting changes in wound status and adjusting treatment plans as necessary.
- Provide education and training to nursing home staff on wound prevention, early detection, and appropriate interventions.
- Collaborate with the interdisciplinary team, including physicians, physical therapists, occupational therapists, and dietitians, to coordinate care and ensure a holistic approach to wound management.
- Assist in the development and implementation of policies and protocols related to wound care.
- Maintain accurate and up-to-date documentation of wound assessments, interventions, and resident responses.
- Stay informed about current wound care best practices, trends, and new treatment modalities through professional development activities.
- Adhere to infection control practices and maintain a clean and safe environment for wound care procedures.
- Provide emotional support and reassurance to residents and their families regarding the management of wounds.
- Weekly rounds with the wound Doctor.
- Valid registered nurse (RN) license in NY.
- Minimum of 2 years of experience as an RN wound care nurse, preferably in a long-term care or nursing home setting.
- Certification in wound care, such as the Wound, Ostomy, and Continence Nursing Certification Board (WOCNCB) certification, is highly recommended.
- In-depth knowledge of wound assessment, staging, and treatment modalities.
- Familiarity with various wound care products, dressings, and equipment.
- Strong critical thinking and problem-solving skills.
- Excellent communication and interpersonal skills to collaborate with residents, families, and the healthcare team.
- Ability to work independently and manage multiple priorities.
