• Conduct comprehensive client assessments to create personalized care plans.
• Administer medications, treatments, and nursing procedures as required.
• Monitor clients  health status (vital signs, progress) and adjust care plans as needed.
• Educate clients & families on disease management, medication use, and home safety.
• Perform advanced nursing procedures including wound care, IV therapy, and catheter care.
• Document all care provided in compliance with regulatory guidelines.
• Ensure compliance with state and federal healthcare regulations.
• Provide emotional support and guidance to clients and their families.
• Stay up-to-date with nursing best practices and participate in professional development.

• Active RN License in the state of New York.
• 1-2 years of nursing experience (home health, geriatrics, or similar preferred).
• Strong clinical skills and ability to perform nursing procedures independently.
• Excellent communication skills and the ability to build rapport with clients and families.
• Self-motivated and organized
  , with the ability to work independently in a home care setting.
• Reliable transportation and a valid driver’s license.
• CPR and BLS certification (required).
• Experience with electronic health records (EHR) systems (preferred).
