Wyckoff Heights Medical Center | Main Campus, New York, USA

Under general direction, conducts and coordinates quality assurance/improvement activities, works collaboratively with the medical staff, departments and clinics to establish and manage an effective quality assurance program for the measurement and evaluation of clinical, functional status and satisfaction outcomes, and improvement of services/outcomes. Follows through on projects and activities designed to improve compliance with all of Centers for Medicare and Medicaid Services (CMS), The Joint Commission (TJC) and New York State Department of Health standards and best practice guidelines, as well as those of other accrediting agencies and regulatory bodies.

Performs related duties as required.

Responsibilities
  • Conducts multiple areas of evaluation, analysis, training and education, re: regulatory and compliance/risk management in conjunction with facility quality director and vice president of regulatory services.
  • Performs concurrent review and retrospective in-depth analysis of patient records for purposes of core measure and stroke care performance.
  • Interacts with physicians, nursing, and ancillary staff to promote accurate documentation to assure that evidence based best practices are implemented timely for core measures and stroke patients.
  • Queries the medical staff and other caregivers as necessary via written/verbal communication to obtain accurate and complete physician documentation that supports core measure and stroke care compliance before and/or after discharge.
  • Serves as resource for core measure/stroke care compliance for patients, staff, and physicians.
  • Promotes evidence based clinical practice standards with nursing staff through mentorship.
  • Performs medical staff and clinical staff education on evidence based practices surrounding core measures, stroke care and other quality initiatives.
  • Tracks compliance of measures and provides staff education relating to the externally reported measurements.
  • Assists in the planning and implementation of interventions to ensure evidence-based practices are implemented (i.e. Core Measures, Sepsis, AMI HF, Pneumonia, COPD, Safe Surgery, ED Throughput, VTE, and Global Immunizations and Stroke Care).
  • Works closely with providers, nurses and other members of the organization by demonstrating effective communications, teaching and negotiating skills.
Position Requirements
  • Bachelor Degree in Nursing or related field, required.
  • Master's Degree preferred
  • Current RN registration and license to practice in NY State.
  • Minimum five (5) years of progressive clinical experience in an acute care setting.
  • Clinical Documentation Improvement/Case Management/ Quality Assurance/Improvement experience preferred.
  • Previous experience in chart review required. Understand coding classifications systems such as, but not limited to ICD-9 CM/ICD-10, MS-DRGs & NYS AP-DRGs.
  • Ability to communicate effectively with physicians and other clinical professional staff.
  • Competent computer skills including Microsoft word, excel, and power point. Must have strong analytical skills. Demonstrated ability to conduct and interpret quantitative/qualitative analysis.
  • CCS, C-CDIS, and/or CPHQ with experience a plus.
  • Inpatient surgical Nursing.

Full Time - Regular 37.5 hours
Days:
Mon – Fri / Shift: 7:00am-5:30pm & One Shift: 7:00am-5:00pm

Hourly Rate: $60.5370
