River Spring Certified Home Health Agency provides skilled home services, including nursing, physical therapy, occupational therapy, speech therapy, and social work as well as home health aide services. The Licensed Practical Nurse (LPN) Community Health Nurse delivers skilled, direct patient care in the home setting under the supervision of a Registered Nurse (RN) and/or Physician, following the patient’s plan of care.

• Promote patient health, independence, and quality of life within the comfort of their home.
• Observe, monitor, and report patient changes; administer treatments; and reinforce patient and caregiver education initiated by the Nurse Care Manager.
• Provide skilled, direct patient care in the home setting under RN/physician supervision as outlined in the patient’s plan of care.

• Graduate of an approved nursing school accredited by the National League of Nursing.
• Current, unencumbered NYS LPN License.
• Minimum of 1–2 years of clinical nursing experience, preferably in home health/hospice, acute care, or sub‑acute settings.
• Ability to prioritize tasks and multitask efficiently and effectively.
• Excellent verbal and written communication skills with the ability to convey patient information clearly.
• Perform weekend on‑call duties as assigned.
• Perform other duties as assigned.
• Knowledge of nursing and medical practices, procedures, regulations, and guidelines pertaining to homecare.
• Valid driver’s license.

• Annual salary: $80,000 – $85,000
