Salary: $135,000 - $150,000/year
Greenlife Healthcare Staffing is seeking a compassionate Family or Adult Geriatric Nurse Practitioner (NP) to join a nursing home in Brooklyn, NY. This role is open to new graduates and experienced NPs, offering flexible scheduling and the opportunity to deliver patient-centered care to elderly residents in a collaborative, mission-driven environment.

Proficiency in EHR systems, chronic disease management, and infection control protocols. - Soft Skills: Strong communication, empathy, cultural competency, and attention to detail.
- Conduct comprehensive patient assessments, including medical history reviews and physical exams.
- Develop and update individualized care plans for residents with acute/chronic conditions.
- Monitor and manage infections by adhering to facility protocols.
- Educate patients and families on disease management, prevention, and wellness.
- Collaborate with physicians, nurses, and therapists to ensure coordinated care.
- Maintain accurate documentation of treatments, progress, and care plans.
