Advanced Practice Provider (Astoria, Queens, NY) – Ennoble Care

Join Ennoble Care as an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to provide primary and palliative care to homebound and palliative care patients in the Astoria, Queens, NY region. You will apply medical techniques and principles under the supervision of a physician and care for a diverse set of patients with chronic diseases.

• Provide high quality primary and palliative care to patients in and around Queens, NY
• Assess patients by performing physical examinations; obtain and update medical histories and develop the best plan for care
• Communicate effectively with office staff and clinical operations across internal platforms
• Deliver patient-centered care to a defined panel of patients
• Attend quarterly medical staff meetings
• Develop impressions, diagnoses, and treatment plans
• Foster trust and consult with patients, families, and caregivers during visits
• Order diagnostic testing as medically necessary
• Prescribe medications, supplies, and appropriate medically necessary interventions

• Board Certified with a valid NY NP or PA license
• Strong clinical skills to accurately assess, diagnose, and treat
• Positive demeanor and strong people skills
• Ability to problem solve and operate independently
• 1-2 years of primary care experience with geriatric patients; hospice experience is a plus but not required

• Salary: $160,000+ depending on experience and incentives
