We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) for a 13-week assignment in a Skilled Nursing Facility (SNF). This position offers a guaranteed 40 hours per week, with variable shifts that may include day and evening hours.

• Deliver direct patient care in accordance with the plan of care under the supervision of an RN or physician
• Administer medications and treatments as prescribed
• Monitor and document patient conditions and report changes to the healthcare team
• Provide support with activities of daily living (ADLs)
• Collaborate with interdisciplinary team members to ensure quality patient care

• Active LPN license in state of assignment
• Minimum of 1 year of recent LPN experience, preferably in a SNF or long-term care setting
• BLS certification (AHA preferred)
• Strong communication and organizational skills
• Ability to work flexible shifts as scheduled
