Base pay range$50.00/hr - $125.00/hr

. This is fee-for-service position.
Pay:
Oasis visit- $100.00 per visit with an extra $25 bonus if notes are completed within 24hrs;
Follow-up/ Revisit- $50.00 per visit with an extra $25 bonus if notes are completed within 24hrs

As a Registered Nurse with us, you'll play a vital role in planning, organizing, and delivering home care services. We're seeking experienced nurses with a passion for community health education and engagement. You'll utilize community resources to tailor services that meet the unique needs of individuals and families within their homes and communities.
- Assessment and Care Planning:
Conduct initial and ongoing assessments of patients' health status. Develop and implement individualized care plans based on these assessments to meet the patient's needs. - Medication Administration:
Administer medications and treatments as prescribed by the physician. Monitor and assess the patient for any adverse reactions or side effects. - Wound Care:
Provide wound care management, including dressing changes, infection control, and monitoring healing progress. - Patient
Education:
Educate patients and their families about their medical conditions, medications, and treatment plans. Teach them techniques for self-care and management of their health at home. - Monitoring and Evaluation:
Monitor patients' progress and condition regularly. Evaluate the effectiveness of the care plan and make necessary adjustments based on patient outcomes. - Collaboration:
Coordinate care with other healthcare professionals, including physicians, therapists, social workers, and home health aides, to ensure comprehensive and integrated care for the patient. - Documentation:
Maintain accurate and up-to-date medical records and documentation of nursing assessments, interventions, and patient responses. - Support and Advocacy:
Provide emotional support and advocacy for patients and their families. Ensure patients' rights and preferences are respected throughout their care.
