Registered Nurse (Nurse Family Partnership) Mother Baby (Spanish Speaking) VNS Health
Registered Nurse role providing high‑quality, comprehensive nursing care for pregnant and postpartum first‑time mothers and infants under the Nurse‑Family Partnership (NFP) program. Responsibilities include delivering direct care, health education, social support, and community linkages while adhering to NFP national guidelines.
Responsibilities- Documents all services provided in accordance with government regulations, VNS Health policies, and NFP requirements.
- Initiates and maintains written and verbal communication with team members and management to assure optimal patient/client care.
- Participates in the orientation of selected staff via a “buddy system” and provides feedback during the orientation process.
- Provides guidance, training, and mentoring to students in selected programs.
- Delivers nursing care in accordance with VNS Health policies, which may require standing, stooping, sitting, crouching, bending, and stretching.
- Travels to patients’ homes and other facilities with varying environments, using approved transportation.
- Follows work procedures in relation to established workflows.
- Maintains productivity sufficient to meet VNS Health goals.
- Assumes responsibility for personal professional growth through in‑services, training, formal education, and professional reading.
- Performs other related duties and participates in special projects as assigned.
- Establishes, implements, and revises a plan of care with input from the client and multidisciplinary team members based on mutually acceptable goals.
- Manages, plans, coordinates, and executes the client’s safe discharge, ensuring discharge summaries comply with VNS Health and regulatory requirements.
- Maintains a flexible work schedule to meet client needs.
- Participates in all required NFP training sessions.
- Participates in special projects and performs other duties as assigned.
- License and current registration to practice as a registered professional nurse in New York State.
- Valid driver’s license or NYS Non‑Driver photo (may be required).
- Bachelor’s Degree in Nursing.
Minimum of one year of recent experience in mother‑child health.
