RN Registered Nurse - Home Care, CHHA – Personal Touch Home Care

$50.00 per day – $125.00 per day

$100.00 per Oasis visit with an additional $25 bonus if notes are completed within 24 hours; $50.00 per follow‑up/revisit with an additional $25 bonus if notes are completed within 24 hours.

As a Registered Nurse, you will play a vital role in planning, organizing, and delivering home care services. You will be responsible for assessing patients, developing care plans, coordinating with other healthcare professionals, and providing education and support.

• Assessment and Care Planning:
  Conduct initial and ongoing assessments of patients' health status; develop and implement individualized care plans.
• Medication Administration:
  Administer medications and treatments as prescribed; monitor for adverse reactions.
• Wound Care:
  Provide wound care management, including dressing changes and infection control.

  Educate patients and families about medical conditions, medications, and self‑care techniques.
• Monitoring and Evaluation:
  Monitor patient progress; evaluate effectiveness of care plans and adjust as needed.
• Collaboration:
  
  Coordinate care with physicians, therapists, social workers, and home health aides.
• Documentation:
  Maintain accurate, up‑to‑date medical records of assessments and interventions.
• Support and Advocacy:
  Provide emotional support and advocate for patient rights and preferences.

• Graduate of an approved school of nursing or Bachelor's degree in Nursing from an accredited college/university.
• Currently licensed, in good standing, to practice in New York as a Registered Nurse.
• Minimum 1 year community health nursing experience or equivalent professional experience.
• Knowledge of nursing and medical practices, procedures, regulations, and guidelines pertaining to home care, including OASIS.
• Compliance with accepted professional standards and practice.
