* Graduate of an accredited school of nursing, Current NYS RN License required.
* Previous RN experience is preferred. New Graduate RN's welcome to apply.
* Previous Home Health Care experience preferred.
* Minimum 1 year of medical-surgical experience.

* Primary Responsibilities:

Planning, coordinating, providing and documenting the care of patients in their place of residence.
* Clinical practitioner who coordinates and implements patient care specific to Jefferson County.
* Ensures that compassionate, engaged care meets or exceeds quality standards.
* Care is provided in an efficient and safe manner consistent with the NYS licensure and Samaritan Home Health standard of care.
* Utilizes nursing skills to determine the biophysical, psychosocial, environmental, self-care, cultural, ethnic and spiritual and educational needs of the patient/family/significant other to develop, implement and revise the plan of care. In doing so, performs accurate assessments, interprets and correlates finding, establishes plan of care and implements short and long-term goals.
* The nurse observes patient progress, records pertinent observations, implements physician and nursing treatment plans and evaluates outcomes.
* The RN recognizes their responsibilities as specified by law and abides within those responsibilities.
* Provides direct supervision to LPNs and HHAs.
* Provides on-call services as required.
* BLS required.
* ACLS preferred.
* Valid NYS Driver's license and proof of auto insurance.
* Must be available weekends and Holidays and provide on-call services as required.
