Job Title: Registered Nurse ACT Team NYC (1 day hybrid)
Location: Brooklyn, NY
Employment Type: Contract M-F, 09:00am-05:00pm with One Hybrid Day
Compensation: $50-55 per hour

- Provide medication education, administration, and monitoring to ACT clients.
- Conduct comprehensive nursing assessments, focusing on both physical and mental health needs.
- Work closely with psychiatrists, therapists, case managers, and peer specialists to develop and implement individualized treatment plans.
- Support clients in managing chronic health conditions and accessing medical care.
- Offer health education on topics such as medication adherence, nutrition, diabetes management, and harm reduction.
- Participate in daily ACT team meetings and interdisciplinary care planning.
- Respond to crises and provide on‑call support as needed.
- Ensure compliance with state and federal regulations, maintaining accurate clinical documentation.
- Licensed Registered RN within the state of New York.
- Must have excellent verbal, written, and interpersonal communication skills.
- Experience working with behavioral health and substance abuse clients (ideal).
- Experience working in ACT, mobile crisis, or forensic mental health settings.
