Licensed Practical Nurse (LPN)

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team in providing quality home care services to patients in the five boroughs of New York. The LPN will be responsible for administering medication, providing wound care, and carrying out various nursing procedures to ensure patients' health and well-being. The LPN will work 12-hour shifts, and additional hours will be compensated accordingly.

Responsibilities
  • Administer medication as prescribed by the physician and monitor patients for adverse reactions.
  • Provide wound care and perform various nursing procedures, including catheterization and tracheostomy care.
  • Monitor and document vital signs, including blood pressure, temperature, and pulse.
  • Maintain accurate and timely documentation of all nursing care provided.
  • Collaborate with the healthcare team to develop and implement care plans.
  • Educate patients and their families on their care plan and medication regimen.
  • Provide emotional support to patients and their families during the course of care.
Qualifications
  • Current New York State LPN license in good standing.
  • Minimum of one year of experience as an LPN in a clinical setting, preferably in home care.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Reliable transportation and a valid driver's license.
  • Ability to lift up to 50 pounds.
