Nurse Practitioner – Johnstown, NY – IDS Health, Inc
We are seeking a compassionate Nurse Practitioner (NP) for a Skilled Nursing Facility in Johnstown, NY.
Responsibilities- Full‑time position with flexible options, working Monday–Friday with occasional weekends and holidays.
- Collaborate with healthcare teams to provide high‑quality care, including assessments, treatment plans, and medication management.
- Manage acute and chronic conditions, document in EHR, and participate in team meetings and care coordination.
- Ensure compliance with regulations and contribute to quality improvement activities.
NY NP license, AGNP/FNP certification, DEA registration, BLS (ACLS preferred), and 1–2 years of geriatrics experience.
